Lockout/Tagout – Devices

  • Locks will be assigned to each employee in the following manner, six locks and keys per maintenance employee (red colored), two locks and keys per operator (yellow colored) and twelve locks and keys per supervisor (green colored). There will also be 12 “equipment locks” per maintenance supervisor (Black  color).
  • Tags and nylon cable ties are to be provided to all authorized personnel. The tags and nylon cables are not reusable. Production operator tags are reusable for exclusion lock-outs (see Exclusions).
  • For normal group lockout steps a hasp is to be connected to the energy isolation device so that all workers may attach their personal lock. Hasps will be located in the respective department LOTO&T station.
  • The authorized person must complete the lockout tags in full prior to working on the respective equipment.
  • A key cabinet will be located in the production managers’ office. Spare keys will be located in the key cabinet.  Only the production manager and maintenance/engineering manager are permitted to access this cabinet.
